About The Position

Chestnut Family Health Center is seeking to hire full-time Health Navigator in our Belleville, IL facility! As a health navigator, you will gain skills to help develop your passion in social work, psychology, counseling, and many other behavioral health fields. Our Health Navigators work with patients to navigate the complexities in the world of behavioral health care. A great candidate for this role is someone with prior experience in substance use or mental health disorder and an extensive background working with patients in health care settings. The anticipated starting pay for new hires for this position is between $44,400 - $50,575 annual salary. Requirements

  • Minimum of a high school diploma with 10 years’ experience working in the human services field, health care or another advocacy role; OR
  • A Bachelor’s degree with 5 years’ experience as stated above; a Master’s degree is preferred.
  • Valid driver’s license, private automobile insurance and be insurable.
  • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, an electronic health record (EHR), general knowledge of standard office procedures and use of office equipment (i.e., filing, including electronic, and the ability to operate copier, fax, and scanning equipment).

Responsibilities

  • Help facilitate patient access to quality primary and behavioral health care.
  • Provide clinician support using social work and psychology theories and models to reduce symptoms from DSM-5 diagnosis.
  • Provide educational resources on care and processes with the patients' social service encounters.
  • Screen clients for issues related to Social Determinants of Health and make service referrals.
  • Facilitate communication among patient, family members, and providers.
  • Manage follow up visits.
  • Ensure continuation of care.
  • Link patient to financial resources.
  • Arrange for interpretive language services as needed.
  • Promote the evidence-based recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness supported through social work and counseling practices.
