Healthcare Community Liaison / Referral Marketer
About The Position
Assisting Hands Home Care East Charlotte is growing, and we are looking for a relationship-driven Healthcare Community Liaison who understands the power of trust, consistency, and personal connections within the healthcare community. This role is ideal for someone who already has experience building referral partnerships with hospitals, assisted living communities, independent living communities, rehabilitation centers, and skilled nursing facilities . We are not looking for someone to sit behind a desk. We are looking for someone who thrives in the field , building meaningful professional relationships that lead to trusted referrals. Requirements
- Has previous healthcare marketing or community liaison experience
- Has established relationships within Charlotte-area hospitals, senior living communities, or rehab facilities
- Is confident walking into facilities and building relationships with healthcare leaders
- Is motivated by growing a territory and generating referrals
- Is personable, professional, and highly relationship-focused
- Understands the post-acute care landscape
Responsibilities
- Developing strong referral partnerships with discharge planners, case managers, social workers, nursing directors, and executive directors
- Visiting hospitals, ALFs, ILFs, SNFs, and rehab facilities throughout the Charlotte area
- Educating healthcare professionals on how our in-home care services support their patients after discharge
- Becoming a trusted resource for facilities that need reliable home care solutions for their residents and patients
- Maintaining consistent follow-up and nurturing long-term professional relationships
- Representing a highly respected and compassionate home care agency in the community
