Schedule: Monday - Friday 9 am-5 pm Location: Field-Based NYC Metropolitan Area Position Summary: As an Account Manager, you will play a key role in expanding True Care’s referral network through strong, established relationships with healthcare facilities and community partners. This role focuses on developing and maintaining referral pipelines with hospitals, skilled nursing facilities (SNFs), nursing homes, rehab centers, and related healthcare organizations , while also increasing brand awareness within diverse communities. Your ability to build trust, navigate healthcare systems, and communicate across cultures will help ensure True Care’s services are accessible, respected, and consistently top of mind.

Requirements

  • 3+ years of experience in community outreach, healthcare sales, business development, or account management
  • Direct experience working with hospitals, SNFs, nursing homes, or healthcare referral partners
  • Demonstrated success in building referral relationships and converting leads into active clients
  • Strong interpersonal, communication, and relationship-management skills
  • High level of cultural competence and experience working with diverse populations
  • Proficiency with Microsoft Office and CRM systems (Salesforce preferred)
  • Valid driver’s license and reliable transportation for local travel

Nice To Haves

  • Russian or Spanish language proficiency strongly preferred

Responsibilities

  • Develop, manage, and grow referral relationships with hospitals, SNFs, nursing homes, rehabilitation centers, and other healthcare providers
  • Proactively identify new referral sources within healthcare and community-based organizations to drive consistent client growth
  • Build and sustain long-term partnerships that align with True Care’s mission and support ongoing referral generation
  • Conduct in-person outreach and relationship management with discharge planners, case managers, social workers, and community liaisons
  • Design and execute outreach strategies tailored to the needs of diverse patient populations, including culturally and linguistically appropriate materials
  • Represent True Care at healthcare networking events, community meetings, and health fairs; serve as a trusted brand ambassador
  • Maintain accurate and timely documentation of outreach activities, contacts, and referrals in Salesforce
  • Collaborate closely with intake, clinical, and operations teams to ensure smooth referral follow-up and client onboarding
  • Support special projects and organizational initiatives as needed
