About The Position

This is a Career Service Health Services Representative position working in the Lee County Department of Health. This Health Service Representative position at the Department of Health in Lee County is under the supervision of the Nursing Program Specialist. The Health Service Representative position involved primarily in the control and prevention of Tuberculosis.  This employee is at risk for blood borne pathogens. This position has access to the following information Sets:  Medical Records, HMS, HIV, Immunizations, Lab, Refugee Health, and Tuberculosis (TB).  This position serves a large population of Spanish speaking clients and employee may be asked to perform interpreter services to facilitate patient services, if bi-lingual. This employee is required to have an annual assessment, which may require a TB screening.  This position works closely with the Senior Community Health Nurse/TB Nurse Case Manager and other team members to ensure all TB patients’ files are opened, followed, and closed appropriately.  Incumbent will be able to perform phlebotomy after completing the appropriate training.

  • Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to threats involving any disaster or threat of disaster, man-made or natural.
  • This position will abide by all state and federal laws, rules, and DOH policies and procedures.
  • Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.

  • This position is classified as sensitive. This individual will perform tasks defined as sensitive and handles confidentialinformation. Background screening is a condition of employment.
  • Selected immunizations or titers and/or periodic screening for tuberculosis may be recommended for this position.
  • This position is authorized to transport confidential information outside of established DOH-Lee facilities and must comply with parameters defined in DOHP 50-10.

Responsibilities

  • Responsible for organization and maintenance of records, filing, scanning, retrieval, and overall maintenance of confidential records for the Tuberculosis (TB) electronic records.
  • This position serves as the referral entry point for TB cases, suspects, LTBI and Interjurisdictional Notifications.
  • Incumbent will register all incoming referrals in HMS and scan any documents into HMS.
  • After registration and scanning, will refer patient to appropriate TB nurse for follow-up and appointment scheduling.
  • Will maintain a checklist of documentation for each referral, assuring completeness of medical records received from referral source and will contact referral source to obtain needed documentation.
  • Provides clerical support for the Refugee Health and TB Programs.
  • May assist refugees to set up appointments within programmatic deadlines and verify refugee documentation.
  • Enters TB services and HMC codes in to HMS.
  • Responsible for Video DOT Program operation.
  • After TB Nurse Case Manager determines eligibility of patients for video DOT and contacts TB Section to obtain state phones for video DOT.
  • Maintains inventory of phones assigned.
  • Reviews patient video DOT, assuring it meets video DOT protocol for completion for both live and store-it -forward video DOT.
  • Documents completion of video DOT in HMS.
  • Trains patients on VDOT setup and troubleshooting.
  • Assists with the expansion of VDOT program as recommended in the manual.
  • Follows up on missed VDOT sessions.
  • Documents VDOT encounters according to IOP 360-02.
  • Tracks medication administration in HMS Medication Tracking module.
  • Responsible for tracking and completion of all TB Contact Investigation documentation in HMS under the guidance of TB Nurse Case Managers.
  • Tracks completeness of Contact Investigation and reports needed actions for each active or suspect case contact investigation at TB Case conferences.
  • Assists in documentation of TB Contact Investigations as directed by TB Nurse Case Manager.
  • Distributes and collects sputum cans, performs TB screening tests, and phlebotomy as directed by TB Nurse Case Manager.
  • Tracks and documents window period testing (8-10 weeks after exposure).
  • Assists with prioritizing contacts (high, medium, low) based on established criteria.
  • Helps schedule prioritized contacts within required timeframes.
  • Supports documentation of contact evaluation outcomes.
  • Maintains contact investigation materials and supplies.
  • Assists with monitoring negative pressure room.
  • Documents daily monitoring of negative pressure.
  • Supports infection control procedures for TB clinic.
  • Maintains infection control supplies (surgical masks, N95 respirators).
  • Serves as a back-up for and assists TB DIS during peaks times with DOT and field visits.
  • Assists with the distribution of TB Control Incentive and Enabler vouchers to patients and orders nutritional supplement supply based on physician’s orders.
  • Maintains inventory of cash equivalents (food certificates, bus tokens).
  • Documents distribution using the Participant Incentive Tracking Log per DOHP 56-89.
  • Prepares and submits Incentive/Enabler Request forms.
  • Coordinates with the CHD business office on I&E accountability.
  • Tracks effectiveness of I&E in promoting treatment adherence.
  • Assists patients with applying for enablers (housing assistance, etc.).
  • Prepares daily report of missed TB clinic appointments.
  • Independently completes routine correspondence for the program.
  • Prioritizes work assignments as required.
  • Communicates and follows administrative and clerical policies and procedures.
  • Provides oversight of TB Program vehicle(s) maintenance needs, overall operability, cleanliness, and communicates with TB Program Supervisor for planning of vehicle budgetary needs for the current and next budget cycle.
  • Maintains the mileage log.
  • Meet deadlines for submission of EAR’s, timesheets, and other items requested.
  • Attends staff meetings and in-service trainings.
  • Performs other duties as assigned.
