Simply put, as the Health Services Director (RN) - Community Based Care your Nursing and Leaderships skills will be used to ensure that quality care is provided to our residents as you direct the day-to-day functions of the Health Services Department. You will be responsible for the delegation of nursing tasks to non-certified staff that work in the Assisted Living and Memory Care communities. As the Health Services Director, responsibilities may include, but are not limited to: Delegate nursing tasks as allowed under applicable state nursing laws and practice acts, communicating regularly with residents, their families, and their health care practitioners. Collaborate with assigned staff and support personnel to plan the shift services, programs, and activities within the facility Participates in the implementation of the resident care planning process to comply with resident needs and regulatory agency requirements. Perform assessments on the health, functional and psychosocial status of each resident. Communicate regularly with residents’ families regarding health care needs. Direct the delivery of in-services/staff development programs that contribute to meeting nursing objectives and standards and comply with all state and federal regulations. Ensures that the department is adequately staffed to provide quality care to residents by participating in the recruitment and selection of personnel. Requirements

  • Must be a licensed RN for the appropriate State.

Nice To Haves

  • Experience as a nursing supervisor is preferred.
  • Assisted Living, Memory Care, Skilled Nursing and/or Home Health Nursing experience is strongly desired.
  • Experience working in mental health sector of healthcare is a plus, but not required.
