Simply put, as the Health Services Director (RN) - Community Based Care your Nursing and Leaderships skills will be used to ensure that quality care is provided to our residents as you direct the day-to-day functions of the Health Services Department. You will be responsible for the delegation of nursing tasks to non-certified staff that work in the Assisted Living and Memory Care communities.

Requirements

  • Must be a licensed RN for the appropriate State.

Nice To Haves

  • Experience as a nursing supervisor is preferred.
  • Assisted Living, Memory Care, Skilled Nursing and/or Home Health Nursing experience is strongly desired.
  • Experience working in mental health sector of healthcare is a plus, but not required.

Responsibilities

  • Delegate nursing tasks as allowed under applicable state nursing laws and practice acts, communicating regularly with residents, their families, and their health care practitioners.
  • Collaborate with assigned staff and support personnel to plan the shift services, programs, and activities within the facility
  • Participates in the implementation of the resident care planning process to comply with resident needs and regulatory agency requirements.
  • Perform assessments on the health, functional and psychosocial status of each resident.
  • Communicate regularly with residents’ families regarding health care needs.
  • Direct the delivery of in-services/staff development programs that contribute to meeting nursing objectives and standards and comply with all state and federal regulations.
  • Ensures that the department is adequately staffed to provide quality care to residents by participating in the recruitment and selection of personnel.
