The Health Record Clerk is responsible for supporting all health record management activities for Family HealthCare Network. This role involves responding to requests for health record and dental information, providing assistance to patients, family members, legal guardians/representatives, and referral sources in accordance with federal, state, and FHCN guidelines. The clerk receives and determines HIPAA compliance for authorizations for release of records, responds to phone inquiries and in-person requests regarding health records, and ensures requests are completed accurately. Key responsibilities also include coordinating with outside copy services, verifying patient demographic information, maintaining health record indexes, and ensuring subpoena requests are met within timeframes. Furthermore, the position supports health care scanning and indexing activities, processes and stores health documents in the electronic health record system, performs quality reviews of scanned images, and updates clinical decision support systems. The clerk is also expected to address and resolve health record issues as they arise, prioritizing requests and problem-solving record locations while providing excellent customer service.
