WI-SSM Health Central Storage Madison Worker Type: Regular Job Highlights: Qualifications: Experience with EHR and FMLA/ Disability strongly preferred Schedule: Monday - Friday 7:30am - 4pm Location: 2202 West Beltline Highway Madison, WI 53713 Job Summary: Responsible for the accurate and time sensitive completion of return to work, Family and Medical Leave Act (FMLA) and Disability related forms received by the Health Information Department.

Requirements

  • High School diploma/GED or 10 years of work experience
  • Two years' experience
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication.
  • Ability to hear alarms, malfunctioning machinery, etc.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.

Responsibilities

  • Reviews and understands patient’s charted medical history, diagnosis and CPT codes to summarize abstracted answers on FMLA and disability forms.
  • Processes disability, FMLA and return to work forms from a variety of employers and insurance companies after the intake process has been initiated.
  • Answers and handles incoming phone calls from patients/customers, insurance companies, employers and clinical staff to provide information regarding workflow and form status.
  • Handles difficult calls in a professional manner and provides solutions to resolve issues in a quick and efficient manner.
  • Validates the authorization to release records to a third party entity; ensures compliance with Health Insurance Portability and Accountability Act (HIPAA) regulations.
  • Scans signed and completed forms and authorizations into the electronic health record.
  • Communicates with clinical staff and providers via electronic health record messages as needed to complete the forms in a timely manner.
  • Reviews finalized forms and ensures HIPAA compliance as well as accuracy and completeness of information provided on the form.
  • Mails, faxes, emails or uses electronic health record messaging completed forms to patient or approved recipients.
  • Performs other duties as assigned.
