About The Position

The position is Grant funded position with an employment term set to end with the grant term on 06/2027. Positions may be extended as more funding is secured. Under the direct supervision of the PHN Supervisor, this position will support, coordinate, and implement grant-funded initiatives awarded to the Navajo Health Foundation/Sage Memorial Hospital under the P4 program. Program areas may include wellness, diabetes, obesity, nutrition, physical activity promotion, gardening, and other public health prevention programs. Requirements

  • Bachelor's degree in health education, Public Health, Nutrition, or a related field and one year of experience in a medical environment in health education, community outreach, or a similar role.
  • Or an equivalent to an associate's degree in health education, Public Health, Nutrition, or a related field and two (2) years of experience in health education, community outreach or similar role.
  • Strong communication and interpersonal skills.
  • Must possess a current American Heart Association BLS certification.
  • Must have a valid state driver's license, Motor Vehicle Report, and maintain a clean motor vehicle record.
  • Must be able to successfully pass the Employee Health Program requirements and background investigation.
  • Comprehensive understanding of nutrition and dietary guidelines.
  • In-depth knowledge of community health education techniques and methods.
  • Expertise in program development, implementation, and evaluation.
  • Proficiency in creating and delivering health education programs and workshops.
  • Familiarity with applicable tribal, federal, state, and local laws and regulations.
  • Awareness of health issues impacting Navajo Nation communities and effective interventions.
  • Principles and practices of health education, social and behavioral sciences, health behavior change, and their applications in public health, mental health, human services, or physical and biological sciences as they pertain to the environment.
  • Effective outreach and community engagement strategies.
  • Strong written and verbal communication skills.
  • Ability to establish and maintain effective working relationships.
  • Proficiency in operating a personal computer and using common software applications.
  • Capability to work independently, meet deadlines, and manage pressure.
  • Demonstrated experience in educating community members and employees through presentations, written materials, and media and social media messaging.
  • Food Service Training Certification

  • Ability to speak the Navajo language and/or familiarity with the Navajo way.

Responsibilities

  • Plan, coordinate, and implement grant-related program activities according to the P4 grant narrative.
  • Cultivate and maintain partnerships with community organizations, businesses, local farmers, and other stakeholders.
  • Develop and implement health education programs to promote healthy eating among participants.
  • Conduct workshops and educational sessions on nutrition, healthy cooking, and the benefits of consuming fruits and vegetables.
  • Provide support and resources to participants to help them understand and utilize their produce prescriptions effectively.
  • Track participant progress and compliance with program requirements.
  • Evaluate the effectiveness of the program and make recommendations for improvements.
  • Collaborate on program development and various activities to achieve grant objectives.
  • Coordinate the purchasing and delivery of produce from local grocers to target communities and participants of the Produce Delivery and Produce Voucher Program.
  • Deliver and assist with the enrollment of P4 participant packets, including commitment agreements outlining the requirements of the P4 Program.
  • Monitor participant compliance, track adherence to program requirements, and address any noncompliance issues, including suspension and/or termination.
  • Coordinate the development of the Nutrition Education curriculum for P4 participants.
  • Provide participants with health education materials and resources for use in the P4 Produce Delivery or Voucher Programs.
  • Provide outcome assessments to include blood pressure, vital signs, surveys, pre-post questionnaires
  • May cross-train to assist with other Community Health nutrition and wellness activities and events
  • Perform additional assigned duties to achieve the goals and objectives of the P4 program.
