About The Position

DUTIES: · Work with adolescents and their families, as appropriate, to connect to ongoing health care and community resources. · Provide on-going health risk assessments, information, and education. · Directly work with youth and their families, including population at greatest risk from disconnection of community resources and supports; independently assess needs of consumers & their families. · Work individually, and as part of a team, to provide group education sessions on various health/teen topics, one on one support when needed, connections to community supports and follow-up. · Assist consumers with sexual health and other various teen health issues through advocacy, accompaniment and general support. · Be able to develop and facilitate daily group activities and program evidence based curriculum. · Develop and maintain effective working relationships with community partners. · Track and compile statistics. · Provide/arrange for training for community partners. · Develop/maintain campaigns to promote community change and increase awareness for adolescent sexual health including teen pregnancy prevention, STD/HIV prevention, and other health topics. · Assist manager in organizing/facilitating partnership meetings, community advisory groups and youth leadership council meetings. Candidates must have a desire to work with individuals and families and have a thorough understanding of the family development process. Candidates must be able to work independently, be self-motivated, resourceful, organized and adaptable in carrying out duties. Must be able to develop and maintain effective working relationships with partners such as reproductive health, social services, educational institutions, parents, other health service providers and the general community. Minimum Qualifications include a Bachelor’s degree or Associates degree and 1-3 years of related experience; or equivalent combination of education, experience, and training. New York State Driver's License with good driving record and access to a reliable vehicle is necessary for reimbursed travel throughout the area. Click here for more info about OCO's Services!
