About The Position

Gardner Health Services is currently recruiting to fill a Health Coach position. This is a full-time, 40 hours/week position based out of the CompreCare Health Center located in San Jose, California. Under the administrative supervision of the Clinic Coordinator and/or Director of Operations, the Health Coach will work closely with the Care Management members to promote habits to improve quality of life and attainment of health goals. The Health Coach is responsible for introducing the Down with Diabetes Program to all eligible Down with Diabetes patients. They will discuss the program with patients, schedule appointments for providers and nutritionists, follow up on missed appointments, invite patients to workshops, distribute program collateral to the patients, and help patients needing assistance learning how to access Zoom or other telehealth technologies. The Health Coach will assist with any other correspondences necessary with the patients to provide case management. The Health Coach will encompass four principle roles: 1. Providing self-management support, 2. Bridging the gap between provider and patient, 3. Helping patients navigate the health care system and offering assistance with appointments with the clinician or the dietician as determined by their needs. Requirements

  • High School Diploma or equivalent.
  • Medical Assistant Certificate from an approved Medical Assistant program as required by GFHN.
  • Experience with EHR and windows operating systems.
  • Exhibits effective communication skills for working with patients, caregivers and healthcare team members.
  • Demonstrates the ability to motivate patients to make critical health changes in their lives
  • Demonstrates a commitment to patients and a willingness to go above and beyond to provide the best care possible.
  • Strong organization skills.
  • Must be bilingual in conversational Spanish (oral and written).

Nice To Haves

  • Experience with diabetes, preferred.
