About The Position

THIS POSITION IS OPEN TO CURRENT OKDHS EMPLOYEES ONLY. This position is located in Tulsa and Oklahoma City, Oklahoma. Health Care Management III - Y15C Annual salary: $69,197.34 + Full state benefits Travel is EXTENSIVE - Must possess a valid driver's license and must maintain required car insurance. Days worked will be Monday through Friday. Minimum Qualifications Health Care Management Nurse, Level III - Requirements consist of possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing and six (6) years of professional nursing experience, including three (3) years in a managed care or home health care program. Preference Statement Preference may be given to applicants with two years of experience working with individuals with intellectual and/or developmental disabilities. Applicants who are QIDP or have experience with CCM. Job Responsibilities: This position directly supervises HCMN II’s who have direct contact with service recipients and their caregivers. Ensure consistency in the training and mentoring of HCMN II’s during their early employment with the Department. Schedule or otherwise facilitate all required DHS/DDS training classes for their assigned staff and assist new HCMN II staff to understand and apply the training in their assigned role. Monitor the case documentation and other assigned tasks required of HCMN II staff they supervise and ensure the timeliness and quality of their work. Coordinate with the Nurse Manger III in community outreach projects and resource development activities, and assist in the development and training of internal and external partners on policies, practices and procedures which may include, but are not limited to: training on contract waiver nursing services; identification of health-related trends in the service population; identifying and implementing service safeguards to assure the delivery of service is compatible with the needs of the individual; evidenced-based best practices for promoting the health of individuals receiving services; assisting service recipients to understand their individual health needs and assessing their ability to seek care when needed; assessing that individuals providing supports to DDS service recipients demonstrate the skills and knowledge requisite to their responsibilities; and ensuring that service recipients are free from abuse, neglect or exploitation. The position analyzes, reviews, and oversees health care services provided by contracted service providers, ensures appropriate health care resources are accessed, and the quality of service meet established standards. This includes working with other DDS staff to review and analyze data related to emergency room visits, hospitalizations, and any unresolved health events for DDS service recipients. Assist with technically complex hospital discharge cases and service recipient transition planning, as well as problem resolution procedures as defined by DDS policy. May assist with the federally mandated Pre-admission Screening and Resident Review (PASRR) process that DDS performs as the State Intellectual Disability Authority.
