Works with the assigned manager/coordinator to ensure consistent and high-quality service is maintained and to identify other opportunities. RN Indiana license or CMA certification required. 1. Actively demonstrates the organization’s mission and core values and conducts oneself at all times in a manner consistent with these values. 2. Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information. 3. Provides wellness education programs on a regular and on-going basis. Provides screenings and education based on the test results. 4. Instructor of health and wellness programs. 5. Participates in program training as prerequisite for each program and demonstrates competency with program material and format. 6. Schedules program classes with manager/coordinator. 7. Keeps attendance list of participants and provides outcome information to designated personnel. 8. Assists in developing new education programs based on community needs. 9. Utilizes effective group leadership skills. 10. Maintains contact with appropriate site staff. Provides outcomes to coordinator or manager. 11. Models appropriate behavior for class participants by a healthy, positive, and upbeat attitude. 12. Seeks answers to participant’s questions and reports back to them. 13. Participates in health fairs, educational presentations and other Community Health and Well Being (CHWB) activities per the request of the director of CHWB. 14. Attends department staff meetings and participates in Process Improvement, if applicable. 15. Utilizes computer programs as appropriate for assigned tasks. 16. Performs other duties consistent with purpose of job as directed.
