About The Position

The Health Access & Outreach Coordinator will contribute to the WellSpace Health Infectious Disease Sexual Health Program through case management of patients, collaboration with community partners, navigation process, data collection, marketing, outreach, appointment scheduling, referral processing, and raising public awareness. As part of the infectious disease sexual health team, this individual will help develop culturally appropriate materials focusing on at-risk individuals who might benefit from sexual health education, screening, treatment, and prevention services. The Health Access & Outreach Coordinator will provide sexual health and infectious disease navigation services (screening, treatment, and prevention), including behavior and harm reduction counseling, resource support and linkage to insurance navigation, and appropriate physician referrals to at-risk individuals.

Requirements

  • High school diploma or GED; 1+ year experience working with LGBTQIA+ or high-risk communities.
  • The position demands a high level of cultural competency in the unique needs of communities of color, particularly those in Hispanic and African American communities, and among the LGBTQIA+ community.
  • Familiarity with HIV prevention, PrEP/PEP, and harm reduction services.
  • The ability to discuss sensitive topics compassionately.
  • Strong communication, outreach, and organizational skills.
  • Proficiency in Microsoft Office and data tracking systems.
  • Willingness to work evenings/weekends for community events.

Nice To Haves

  • Strong community relationships and knowledge of the Sacramento area will be a plus.

Responsibilities

  • Conduct outreach and education at events and in community settings.
  • Manage and process referrals, ensuring timely linkage to care for all patients with positive test results.
  • Monitor referral outcomes.
  • Coordinate linkage to PrEP or Doxy-PEP services for patients.
  • Provide sexual health screenings, referrals, and navigation for HIV/STI care, including collaborating with providers, case managers, and medical assistants.
  • Maintain and regularly update the Master Patient List to ensure all patients currently in care are tracked appropriately and not lost in care.
  • Track patient care plan for chronic infections such as LTBI, Hep C, HIV, and PrEP.
  • Educate clients on prevention options and support care adherence.
  • Collaborate with internal teams and external partners for care coordination.
  • The ability to work independently and collaborate with a team, be self-motivated, and operate in a fast-moving environment is mandatory.
  • Assist clients with insurance enrollment and overcoming access barriers.
  • Maintain accurate records and support program reporting and compliance.
  • Represent IDSH at health fairs and community coalitions.
  • Support marketing, outreach, and patient engagement strategies.
  • Report and present program and patient’s metrics or outcomes at STI Workgroup and Clinical Quality Meetings.
  • Participate in quality improvement processes.
  • Other duties as assigned.
