Requirements

  • A degree in social work, psychology, or a related field is preferred
  • Proven experience in direct care and care coordination
  • Exceptional communication and interpersonal skills are critical for effective collaboration with families, direct support providers, and internal stakeholders
  • Proficiency in relevant software tools is crucial for managing caseloads and ensuring compliance with audit monitoring systems
  • The ability to work independently while fostering teamwork is vital, as you will oversee service provision and guide direct support providers
  • Critical thinking and problem-solving skills will empower you to develop effective Habilitation (Hab) and Attendant Care (ATC) plans, ensuring quality support
  • Flexibility and adaptability in a fast-paced environment will enhance your capability to respond to emergency service requests and promote team health
