About The Position

Residential House Case Manager – Concord, NC Turning Point Family Services is hiring a Case Manager for our Residential Level III home in Concord, NC. This home has been in operation for over two years and successfully serves youth in Mecklenburg County DSS custody. The home provides a safe and therapeutic environment for high-risk youth with behavioral issues while searching for long-term placement. We currently have one full-time position available Mon-Fri. Starting pay is $48,000-$52,000 and is contingent on experience. Position Summary Responsible for overall case management functions for clients in residential level III. Responsible for securing appropriate placement for clients and for overall quality management components of the programs. This position requires a clear understanding of the agency’s services and strategic vision. Importance is placed in creating a culture of compliance and quality service delivery, with an emphasis on teamwork, confidence in service delivery, and customer service.

Requirements

  • Education: Bachelor’s Degree in the Human service field
  • Work Experience: Minimum of 2 years documented experience working with the population served preferred (Must be a Qualified Professional)
  • Knowledge, Skills & Abilities: Knowledge of community resources; strong communication skills; computer proficiency (Word, Excel, e-notes); organizational and prioritization skills; flexibility; ability to multitask; sound judgment; initiative

Responsibilities

  • Facilitate all Child and Family Treatment Team meetings in coordination and consultation with other members of the treatment team (DSS, MCO, etc.).
  • Responsible for securing appropriate placements for clients in coordination with DSS and MCO staff. Serve as QP for Union house.
  • Facilitate and complete intake paperwork/process for new admissions and discharges. Facilitate and complete paperwork (Person Centered Plans, etc.) for all Medicaid Authorizations. Make necessary appointments for clients and assist with transportation to appts.
  • Work in conjunction with Program Manager, Program Director, and QA/QI Director to ensure the facilities and all required documentation (notes, MAR’S, fire drills, etc.) are of the highest quality standard and are reflective of appropriate interventions in the Person-Centered Plans. Conduct routine audits.
  • Review Shift Notes routinely to ensure they are completed accurately and thoroughly with an eye to quality.
  • Complete and submit incident reports for both residential and day treatment (within the crisis programs).
  • Review Incident reports alongside Program Manager to ensure highest quality treatment is delivered.
