Wellness engagement can reduce premiums by nearly $500 Position Overview As a HANDS Specialist I, you will serve as a trusted partner to families with infants and toddlers, supporting healthy child development and empowering parents during the critical early years of life. Through home visits and community outreach, you will educate, connect, and advocate for families, helping them access resources while promoting positive parenting techniques. This is a full-time, non-exempt role ideal for compassionate professionals with backgrounds in social work, nursing, early childhood education, or related fields who are passionate about making a lasting impact on families and young children. In the Health Access Nurturing Development Services (HANDS) program, responsible for reviewing clinic records and interviewing / conducting Parent Visit Assessment/Survey with parents (prenatal and/or immediately after the birth) to determine appropriate level of support from the community

Requirements

  • Education/Experience Bachelor’s Degree in Social Work, Nursing, Early Childhood Education, Social or Behavioral Science, or related field.
  • One (1) year of experience providing case management services required if degree is not directly in Social Work, Nursing, or Early Childhood Education.
  • Substitution for Education Associate Degree in Nursing with current Kentucky (or compact state) licensure and two (2) years of public health experience.
  • Must meet education requirements per 902 KAR 4:120.
  • Current CPR certification (or ability to obtain within probationary period).
  • Strong interpersonal and communication skills, with ability to build trust with diverse families.
  • Knowledge of early childhood development, family dynamics, and public health principles.
  • Ability to maintain accurate documentation and adhere to program guidelines.
  • Commitment to confidentiality, professionalism, and cultural sensitivity.
  • MINIMUM REQUIREMENTS ARE COMPREHENSIVE STATEMENTS OF THE MINIMUM BACKGROUND AS TO EDUCATION, EXPERIENCE, AND OTHER QUALIFICATIONS WHICH WILL BE REQUIRED IN ALL CASES AS EVIDENCE OF AN APPOINTEE'S ABILITY TO PERFORM THE WORK PROPERLY.
  • EDUCATION: Bachelor’s Degree in Social Work, Nursing, Early Childhood Education or a Bachelor’s degree in a Social or Behavioral Science related field may also be considered with one (1) year experience performing case management services.

  • EDUCATION: A Master’s degree in Human Services may substitute for the one (1) year of experience.
  • EXPERIENCE: Associate degree in Nursing with licensure in Kentucky or a compact state and two (2) years of public health experience will substitute for education.

Responsibilities

  • Family Engagement & Support Conduct parent assessments and surveys within 30 days of referral based on prenatal or birth records to determine program eligibility.
  • Provide ongoing home visitation and outreach to support families in developing positive parenting and coping strategies.
  • Educate parents on child development, nutrition, safety, and early education resources.
  • Connect families with community resources and provide referrals as needed.
  • Program Implementation Maintain accurate service logs, schedules, and charting in compliance with HANDS program policy and guidelines.
  • Prepare reports and analyses as required by state and local program standards.
  • Develop and distribute informational materials that meet HANDS program requirements.
  • Interpret laws, regulations, policies, and procedures to ensure compliance with state and federal standards.
  • Collaboration & Professionalism Work closely with supervisors, coordinators, and agency staff to monitor and evaluate program requirements.
  • Participate in program modifications and improvements as needed.
  • Attend meetings, trainings, and seminars to remain current on HANDS standards and best practices.
  • In the Health Access Nurturing Development Services (HANDS) program, responsible for reviewing clinic records and interviewing / conducting Parent Visit Assessment/Survey with parents (prenatal and/or immediately after the birth) to determine appropriate level of support from the community
  • Conduct parent visit assessment/survey within 30 days of receiving, after review of prenatal or birth records from the hospital or clinic, to determine eligibility for early childhood education and family support services of infants and toddlers from birth up to three (3) years of age.
  • Will conduct interviews to promote outreach activities in family homes or community setting to educate parents in coping techniques and to take an active role in accessing resources or assistance with referrals for appropriate education and nutrition of their child.
  • May develop and distribute informational materials that meet program guidelines.
  • Keep accurate logs of service visits, schedules and charting, prepare reports and analyses set forth in HANDS program policy and guidelines.
  • Interpret laws, regulations, policies and procedures and provide administrative assistance or guidance to departmental staff, which may include but is not limited to; monitor, evaluate and participate in modifications regarding program requirements.
  • Review records and reports submitted by program requests or agency staff and ensure compliance with federal and state law regulation and policies.
  • May conduct ongoing home visitation on an as needed basis.
