About The Position

General Duties include, but are not limited to: Family Support Worker I - Conduct regularly planned visits to establish a trusting rapport for eligible HANDS families; Assist parents with child development, parenting skills, health services and other needed resources identified at the initial assessment or during visits; Maintain confidentiality of program information obtained from home visits and exchanges while communicating with community and support service providers; Provide support to parents during the prenatal period and up to, but not inclusive of the child’s third birthday following the initial parent visit assessment; Facilitate family visitations that may take place in the client’s home or another community site, if justified; Monitor and document the progress of the parent and child(ren); Prepare and maintain case records which shall be documented with contacts, services needed, reports and progress; Confer with the Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers. HANDS Specialist I and Public Health Nurse I - Conduct parent visit assessment/survey within 30 days of receiving and reviewing prenatal or birth records to determine eligibility for early childhood education and family support services of infants and toddlers from birth up to three (3) years of age. Will conduct interviews to promote outreach activities in family homes or community setting to educate parents in coping techniques and to take an active role in accessing resources or assistance with referrals for appropriate education and nutrition of their child. May develop and distribute informational materials that meet program guidelines. Keep accurate logs of service visits, schedules and charting, prepare reports and analyses set forth in HANDS program policy and guidelines. Interpret laws, regulations, policies and procedures and provide administrative assistance or guidance to departmental staff, which may include but is not limited to; monitor, evaluate and participate in modifications regarding program requirements. Review records and reports submitted by program requests or agency staff and ensure compliance with federal and state law regulation and policies. May conduct ongoing home visitation on an as needed basis. CHARACTERISTICS OF A CLASS: ARE GENERAL STATEMENTS INDICATING THE LEVEL OF RESPONSIBILITY AND DISCRETION OF POSITIONS IN THAT JOB CLASSIFICATION. Responsible for initiating and maintaining regular and long term contact and provide support to first time voluntary parents during the prenatal period and extending up to but not inclusive of the child’s second birthday. The entry-level incumbent would participate in on the job training programs to become knowledgeable in introducing parenting skills to empower families to make their own decisions while remaining non-judgmental of families they serve.

Requirements

  • Family Support Worker I - High School Diploma or GED.
  • HANDS Specialist I - Bachelor’s Degree in Social Work, Nursing, Early Childhood Education or a Bachelor’s degree in a Social or Behavioral Science related field may also be considered with one (1) year experience performing case management services.
  • HANDS Public Health Nurse I - Must have RN license in Kentucky or compact state or provisional license in Kentucky, meeting all requirements of KRS 314.041 and approval to practice as Registered Nurse Applicant (RNA)
  • Must be 18 years of age with reliable transportation, valid driver’s license and telephone accessibility
  • Formal job training requirements must be successfully completed within 6 months after hire
  • Knowledge of assigned geographic areas and/or ability to locate participants and community resources
  • Must be 18 years of age.
  • Must have telephone accessibility.
  • Must have reliable transportation and valid Driver’s License.
  • Criminal background check.
  • HANDS home visitor core training (Must be completed before family contact can occur)
  • HANDS wrap-around required training (Specific topical areas checklist)
  • CPR Certification
  • Working knowledge of geographic area and or ability to locate participants and community resources with use of maps, contacts, and directions provided.

  • Facilitate families in securing of prenatal/child health services, referrals, and appointments with other supportive agencies.
  • Document case records, home visit logs accurately on each contact, progress, service need and completes reports as required.
  • Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers.
  • Confer with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.
