About The Position

As a HANDS Ongoing Home Visitor, you will work directly with first-time parents and families, beginning in the prenatal period and continuing through the child’s second birthday. By providing regular home visits, you will build trusting relationships, support families in developing parenting skills, and connect them with vital health and community resources. This is a full-time, entry-level role designed for compassionate individuals who want to empower families, promote early childhood development, and make a lasting impact in the community.

Requirements

  • High School diploma or GED required.
  • Must be at least 18 years of age.
  • Must have telephone accessibility, reliable transportation, and a valid driver’s license.
  • Must successfully pass a criminal background check and drug screening.
  • Completion of HANDS Core Training, wrap-around topical training, and CPR certification within 6 months of hire.
  • Ability to navigate within the community to locate participants and resources.
  • Strong communication and interpersonal skills.
  • Ability to remain non-judgmental, empathetic, and supportive when working with diverse families.
  • Knowledge (or willingness to learn) about infant/toddler development, parenting skills, and family support.
  • Commitment to confidentiality and public health best practices.

Responsibilities

  • Family Engagement & Support Conduct initial and ongoing home visits with eligible first-time parents and families.
  • Establish rapport and provide education on child development, parenting, nutrition, and health services.
  • Encourage parents to take an active role in accessing resources and making informed decisions for their child’s future.
  • Assist families in securing prenatal care, pediatric services, and referrals to supportive agencies.
  • Provide guidance on coping skills, safety, and early childhood milestones
  • Program Implementation & Documentation Maintain accurate case records, visit logs, and reports in compliance with HANDS program standards.
  • Participate in on-the-job training, including HANDS Core Training, wrap-around training modules, and CPR certification within 6 months of hire.
  • Protect the confidentiality of program participants and information exchanged with service providers.
  • Confer with supervisors or program coordinators on complex cases or collaborative service needs.
  • Collaboration & Community Engagement Partner with community organizations, clinics, and agencies to connect families with additional support.
  • Provide outreach and educational materials in alignment with HANDS guidelines.
  • Represent the HANDS program positively in the community.
  • Responsible for initiating and maintaining regular and long term contact and provide support to first time voluntary parents during the prenatal period and extending up to but not inclusive of the child’s second birthday.
  • Conduct initial home visit following Parent Visitor assessment with first time parent/s and or families to establish rapport and planning for providing services as qualified by the provider.
  • Provide regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant/s to assist first time parent/s and or families with child development, parenting skills, health services and other needed resources identified at assessment or during visits.
  • Facilitate families in securing of prenatal/child health services, referrals, and appointments with other supportive agencies.
  • Document case records, home visit logs accurately on each contact, progress, service need and completes reports as required.
  • Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers.
  • Confer with Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.
