About The Position

Mosaic is seeking a Habilitative Specialist/Case Manager to coordinate services. This role is envisioned as a "Program Captain" who ensures services are delivered effectively and in compliance with all necessary rules. The ideal candidate is a solutions-oriented problem solver with a thorough and conscientious approach, an efficient helper who can smooth bumpy situations, and a natural teacher who provides tangible solutions and helps others connect the dots. The position involves training staff, developing new programs, and maintaining records of the individuals served.

Requirements

• A Bachelor’s Degree in Psychology, Special Education, or a related Human Services field OR 5 years relevant work experience in lieu of the degree
• A minimum of one (1) year of experience working directly with individuals with developmental or intellectual disabilities
• Proven ability to ensure program delivery is in compliance with regulatory requirements

Nice To Haves

• Previous experience working within an Home and Community Based Waiver (HCBS) setting
• Strong organizational skills

Responsibilities

• Train staff in behavior modification techniques
• Develop brand-new programs and strategies to help individuals learn, grow, and hit personal goals
• Maintain records, journaling the success stories of the people whose lives are being transformed
