As our Group Leader, you will provide individualized self-management education and training to both individuals and groups, functioning in a non-technical role to deliver instruction to program participants, all in alignment with the organization's mission and core values. Every day you will educate and train individuals and groups on self-management techniques according to your scope of practice, ensuring that your behavior consistently reflects our core values. To be successful in this role, you will demonstrate excellent communication and instructional skills, a commitment to supporting program participants in self-management, and a deep understanding and active participation in fulfilling the mission of the organization. Able to lead, facilitate and manage groups of students and participants in Community Health programs with effective communications skills and professionalism. Utilize evidence based practices as indicated in curriculum being presented for each program and maintain fidelity of each program. Develop a collaborative, therapeutic alliance with participants and community members. Identify when a program participant's needs are outside the scope of the group leader's expertise and has the ability to arrange for additional services to meet needs. Provide outreach to older adults and work with community based groups. Make oral presentations on health topics and programs details in community settings. Attends/participates in departmental meetings including staff meetings and in-services. Participates in departmental and interdepartmental work teams.
