About The Position

Easterseals Blake Foundation started as the Cerebral Palsy Foundation of Southern Arizona. Since then, we have expanded our services to better serve our community members. We are now one of Arizona's top providers of comprehensive behavioral health services, child welfare, and services for individuals with intellectual and developmental disabilities. We serve more than 40,000 individuals and families across 10 counties and support more than 3,700 children in Foster Care throughout Southern Arizona. General Position Description: Responsible for the coordination of behavioral health services, communication between the member and other providers or community agencies to ensure coordination of care, facilitation of structured children’s and parenting group services and interventions, skills training, and crisis intervention.

Requirements

  • High School/GED with 1 one year of behavioral health or related work/volunteer experience required.
  • Must be at least 21 years of age.
  • Current, unrestricted AZ driver’s license with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.
  • Must be able to pass a criminal background check.
  • Ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card (employer provides).
  • CPR and First Aid certification (Employer provides).
  • Satisfactorily Completed Competency Checklist within 6 months of hire.
  • Must have a strong clinical knowledge of development across the life span, and knowledge of best practices when working with diverse populations with trauma backgrounds.
  • Extensive knowledge of managed care systems, AHCCCS/Title XIX medical assistance screening, and ADHS policies and practices.
  • Able to establish and maintain a team atmosphere of communication and collaboration for all that reach out to the Department.
  • Must be self-directed and be sensitive to cultural and linguistic diversity.
  • Must be able to utilize DSM-5-TR to provide diagnoses for older children, adolescents and adults and the crosswalk to the DC-0-5 to provide diagnoses or diagnostic impressions for young children.
  • Must be able to remain calm and non-defensive with a supportive attitude during stressful, potential crisis, and crisis situations.
  • Must be able to work in an extremely stressful environment where there are considerable mental and emotional demands.
  • Must be able to manage high volumes and unpredictability of work.
  • Must be able to remain flexible and reprioritize according to the changing demands of the day.
  • Maintains a professional in appearance, communications, and actions.
  • Be able to use excellent interpersonal communication skills.
  • Possess a high level of computer proficiency.

Responsibilities

  • Facilitates parenting and children’s groups using an evidence-based curriculum. Provides direct care and supervision of clients in a group setting.
  • Assists in developing activity plans and preparation for each group/session.
  • Provides meaningful teaching opportunities, experiences, and physical assistance for groups or individuals, including social skills, self-care, and recreational activities.
  • Screens, evaluates, schedules, supports, and continually evaluates referred members in need of behavioral health services.
  • Researches eligibility for behavioral health services and assists members in securing necessary behavioral health services from EBF or other appropriate providers.
  • Assists in engaging caregivers/children in defining issues, creating goals, and identifying strengths to meet their clinical needs and objectives.
  • Provides support services to enrolled members, including case management, personal assistance, family support, peer support, respite care, transportation, and other services.
  • Recognizes, assesses, prevents, and responds to a crisis. Completes safety planning and risk assessments as needed.
  • Completes progress notes in Sigmund within 24 hours of service.
  • Communicates with the guardian, member, and/or any other team member to initiate/coordinate services.
  • Attends and participates in Child and Family Team and Adult Recovery Team meetings as needed/required.
  • Participates in the process of ongoing professional development and meetings. Maintains comprehensive communication among all contacts within the agency, members, and external agencies. Models professional standards and ethics in accordance with EBF Code of Conduct, Strategic Plan, Mission, and Vision.
  • Performs other duties as assigned.
