Under the general direction of the Counseling Center clinical staff, the case manager promotes holistic student wellness, by meeting students individually or in other forums (classrooms, division meetings, programming) and helping students gain awareness of and access to solutions to their barriers to academic success. This position requires a variety of skills involving case management, crisis intervention, triage, outreach, and consultation. Case management may include linking with providers and maintaining lists of relevant referral sources, supporting treatment planning efforts, coordinating care needs, navigating on campus barriers and submission of documents (financial aid, emergency withdrawal, post-hospitalization success planning, accommodations with disability support), conducting follow-ups to ensure referral connection was made/care received and assist in removing barriers to care.
