Job Summary This position is responsible for contacting members and community partners to support healthcare initiatives and activities including educational and quality campaigns, assistance in scheduling care manager and member appointments. This position is also responsible for performing ongoing functions related to member health-related social needs directly or via social service department and community-based organizations. The position responds to inquiries from members and supports the clinicians in the physical and behavioral health care management department with their provider and member activities. This position is also responsible for supporting relationships with community partners, working with community-based organizations, and contributing to the design of community health outreach programs. This position may require field-based work commensurate with initiatives, health campaigns and departmental function. This position may also be responsible to manage low-risk members for health-related social needs, education, and basic member outreach screenings, assessments, and inquiries.

Requirements

  • Bachelor Degree OR 1 year of college and 3 years of health, social service or community experience, OR 4 years' health, social services or community experience,
  • Knowledge of medical terminology
  • Experience coordinating member related needs, providing assistance to members, and analyzing member needs
  • PC proficiency including Microsoft Office applications
  • Customer service skills
  • Verbal and written communications skills including developing written correspondence to members and to other department personnel and coaching skills, including motivational interviewing, to educate members on medical and BH issues
  • New Mexico Only: Bachelor's degree in Social Work Proficient in New Mexico Native American Pueblo language; must be able to translate and present
  • Community Health Worker (CHW) certification
