Serves as part of a multidisciplinary team to seek out, coordinate, connect and navigate all students and families to supports and resources. Provides direct mental health services to general education students to meet needs and support student achievement. Develops partnerships both within the school and community and ensures that students and families receive access to human services, health care, social services, and mental health supports. Essential Duties and Responsibilities: An employee in this position may be called upon to do any or all of the following. (Does not include all tasks an employee may be expected to perform; other duties may be assigned.) Serve as an initial point of contact for students and families in order to access mental health support and other services. Conduct home visits as appropriate. Administer screening and assessments to identify social, emotional, and/or mental health needs and to determine Plan of Care eligibility. Provide mental health support in the form of brief interventions and therapeutic modalities. Coordinate care with community resources and schools to assist families in accessing services and navigating systems. Advocate for appropriate access to human services, mental health and medical health services. Promote and foster communication between school staff, students, families, and service providers. Collaborate with JCISD Attendance and McKinney-Vento staff to coordinate services for identified students. Design and implement strategies to engage students and families, foster positive school culture and climate, raise awareness of non-academic needs, and connect families with school and community resources and supports. Attend multidisciplinary team meetings as appropriate. Support and facilitate implementation of Multi-tiered Systems of Support (MTSS) tiered interventions (i.e., psychoeducation, tier two groups, and positive behavior supports). Consult, as appropriate, with building/district staff on classroom and/or building strategies to meet student needs. Utilize required data systems to document student progress, write regular progress reports, and complete all required documentation in a timely manner. Collaborate with Whole Child team to develop, coordinate, and implement professional learning opportunities. Fulfill all Medicaid billing requirements. Participate on and collaborate with Whole Child and Learning Services teams and other committees as appropriate. Attend other required meetings. Regular and predictable attendance is required for this position. In-person attendance requirements may vary and will be determined by the supervisor. Work location may vary. Travel is required to assigned locations. Adhere to the Michigan Code of Educational Ethics, the Michigan Mental Health Code, and the National Association of Social Workers Code of Ethics. Perform related work as required/ other additional duties as assigned.
