About The Position

This is not a traditional Medical Director role. This is for a physician who believes that presence matters as much as protocol. At Milestones, our clients are navigating profound trauma, complexity, and vulnerability. The Medical Director is not just overseeing care. They are woven into the therapeutic experience, partnering closely with clinicians, guiding nuanced medical decisions, and helping shape a treatment environment where clients feel deeply seen, safe, and supported. As our Fractional Medical Director, you will bring high-level psychiatric and medical expertise into a high-touch, highly relational treatment model, working 10–15 hours per week in close collaboration with our clinical and therapeutic teams. Your impact will be felt in the moments that matter most: When a client is in crisis and needs thoughtful, grounded medical leadership When a therapist needs a collaborative partner to navigate complexity When the team needs alignment around safe, effective, and human-centered care This role is ideal for a physician who wants to stay deeply engaged in meaningful clinical work, without the demands of a full-time administrative structure, while still playing a critical leadership role in client outcomes and program integrity.

Requirements

  • Board-certified Physician with an active Tennessee license
  • Experienced in trauma, behavioral health, or residential treatment settings
  • Clinically sharp and comfortable navigating high-acuity, complex cases
  • Highly relational, collaborative, and emotionally intelligent
  • Able to balance clinical authority with humility and partnership
  • Drawn to mission-driven work where client transformation is the priority

  • Psychiatrist strongly preferred

Responsibilities

  • Provide medical oversight for a high-acuity adult trauma treatment program
  • Serve as a key clinical thought partner for therapists, psychiatrists, and nursing staff
  • Support complex case consultation, risk assessment, and clinical decision-making
  • Guide medical best practices in alignment with evolving psychiatric and trauma-informed care standards
  • Actively engage with the multidisciplinary team in a highly collaborative, non-siloed model of care
  • Build trust and credibility with clinicians through accessibility, humility, and partnership
  • Participate in case reviews, team consultations, and clinical discussions
  • Maintain a high-touch presence with clients as needed for assessments, medication oversight, and clinical support
  • Contribute to a treatment environment that prioritizes dignity, emotional safety, and whole-person care
  • Support crisis situations, including higher-risk or complex psychiatric presentations
  • Partner in the evolution of clinical programming and medical integration
  • Ensure alignment with regulatory, accreditation, and best-practice standards (CARF, HIPAA, etc.)
  • Help maintain a consistent, high-quality standard of care across the program
