About The Position

We are seeking a compassionate and organized Foster/Adoptive Care Case Manager to support foster and adoptive families and help ensure children receive safe, stable, and trauma-informed care. This role involves home visits, school visits, coordination with community partners, and participation in treatment team meetings. The position covers Lincoln, Boone and surrounding counties. The home office would be our Lincoln Office.

Requirements

• Bachelor’s degree in Social Work or related human services field
• Minimum two (2) years experience working with children and families
• Valid driver’s license, reliable transportation, and auto insurance
• Strong communication, organization, and documentation skills

Nice To Haves

• Social Work license through the West Virginia Board of Social Work Examiners preferred but not required
• Knowledge of WV Medicaid and child welfare systems preferred

Responsibilities

• Conduct home visits (minimum twice monthly) and school visits (minimum monthly)
• Participate in MDT meetings, IEP meetings, court hearings, and treatment team meetings
• Provide supervised visitation and document services in compliance with Medicaid and agency standards
• Maintain accurate and timely documentation
• Collaborate with families, schools, DHHR, and community partners
• Monitor child safety and well-being
• Participate in supervision, training, and on-call rotation
