About The Position

The incumbent of this position is responsible for case management duties associated with offenders with chronic illnesses, mental illnesses, and/or substance abuse disorders. Work is performed under the general direction of the Director of Mental Health This is a "mandatory testing" position that requires Drug Testing.

Requirements

  • Bachelor's Degree from an accredited college or university in Counseling, Criminal Justice, Psychology, Social Work, or a related field
  • Must possess a Cardio-Pulmonary Resuscitation (CPR) certification at the time of employment.

  • During emergency conditions, the incumbent of this position will automatically be considered an emergency service worker. This employee is subject to being called to work in the event of a disaster, such as a hurricane, or other emergency situation and is expected to perform emergency service duties as assigned.
  • Positions in this class typically require fingering, grasping, talking, hearing, seeing and repetitive motions.
  • Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for Sedentary Work and the worker sits most of the time, the job is rated for Light Work.
  • Incumbents may be subjected to blood and other bodily fluids.

Responsibilities

  • Works closely with jail mental health staff and community partners to ensure patients' mental health status is maintained appropriately during incarceration
  • Provides linkages between community providers/partners, jail health staff, and others as warranted to share information related to previous mental health treatment and medications
  • Works closely with the jail court liaison during judicial proceedings to provide reports and information relevant for diversion and other possible needs. Informs jail court liaison and makes recommendations for possible alternatives to incarceration
  • Performs all case manager duties as necessary during and post-incarceration to ensure continuity of care and ensure smooth transition of patients back the appropriate level of care
  • Responsible for complete and accurate documentation of care provided in the inmate's health record
  • Maintains health services records, logs, and assists with data entry
  • Consults with other practitioners and related staff, as appropriate in the performance of casework
  • Attends treatment team meetings for information sharing and coordination of services
  • Follows all standards and patient care protocols, all federal, state, correctional health care standards, and the Escambia County Board of County Commissioner's policies, regulations, and guidelines
  • Performs other duties of a similar nature or level
  • Performs other related duties as assigned
