About The Position

Helio Health is seeking a Forensic Case Manager to support individuals recovering from substance use and mental health disorders. This full-time role is essential for managing and coordinating services for patients in the Forensic Evaluation Unit Program, including conducting intakes, making referrals, and reporting progress to the court.

Requirements

• Associate's degree in a related human services field required; Bachelor's preferred.
• Two years of experience working with behavioral health and/or substance abuse issues.
• Valid NYS Driver's License preferred.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including screening, intake, and orientation.
• Makes referrals to appropriate service providers.
• Provides counseling and crisis intervention.
• Educates patients/residents about their treatment.
• Manages cases and maintains accurate reporting and record keeping.
• Consults with other professionals and coordinates care with community providers.
• Plans for patient discharge and follow-up care.
• Trains staff, volunteers, and students/interns.
• Performs quality assurance activities and monitors service provision.
• Evaluates programs to identify strengths and weaknesses.
• Assists in problem resolution and implements approved program modifications.
• Participates in program administrative and clinical committees as assigned.
• Represents the agency to the community.
• Performs other duties as assigned by supervisor.
