In 2022, amid rising food insecurity rates, Hartford Hospital launched the Food4Health program, an umbrella program that includes a food clinic supported through community partnerships and on-site hydroponic farms that generate thousands of pounds of fresh produce each season. People experiencing food insecurity – 100% have moderate or high food insecurity - and health conditions that benefit from healthier diets, receive prescriptions to pick up healthy food items for themselves and their families at the clinic weekly. The Food4Health program is available for patients in 24 clinics who meet the criteria for inclusion as well as Hartford Hospital staff who need assistance. The Community Health Worker (CHW) works to promote, maintain, and improve the health of underserved residents and their families by providing social support and basic information to promote healthy behaviors. This position uses their cultural connectedness to the community to provide information, advice, and support related to healthy behaviors and access to the Food4Health program. The Community Health Worker (CHW) works as a member of the Food4Health team through an integrated approach to care management and community outreach. CHWs, are connected to the community they serve and are recognized as a “critical link” to connect underserved residents to beneficial resources and navigate the ever-changing healthcare environment. Specific activities of a CHW include screening clients in the community for Social Determinants of Health (SDOH), helping patients better understand the components and how to best utilize the program, clearing barriers to usage and helping to manage the flow of patients alongside the Food4Health Program Manager.

Requirements

  • High School diploma preferred
  • 1-2 years of experience in healthcare or a community facing community health position
  • Connection to the community being served required.
  • CT CHW Certification or in process of working towards the certification.
  • Bilingual (English and Spanish) reading, writing and speaking is required.
