Care Compass is a new, transformative program. This integrated, regional, team-based model is designed to manage and coordinate care for MGB’s high risk primary care patients. Members of the team include RN Care Managers, Social Workers, Community Health Workers, and Complex Care Coordinators. Job Summary Primary Responsibilities: Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors.

Requirements

  • High School Diploma required; Associate’s Degree or Bachelor’s Degree preferred
  • 1+ year of experience in community outreach, health education, or related field required
  • Bilingual (English/Spanish) preferred
  • Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
  • Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred.
  • Ability to work independently and as part of a team.
  • Basic computer skills, including Microsoft Office and database management.

Responsibilities

  • Conduct community outreach and engage community members to understand their health needs
  • Provide individual and group health education and coaching to community members
  • Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services
  • Provide basic health screenings and assessments to identify health risks and provide appropriate interventions
  • Assist in the development of health promotion programs and activities to address community health needs
  • Collect data and maintain accurate records of community health outreach activities and interventions
  • Attend meetings and trainings related to community health promotion and education
