About The Position

Assists with the provision of patient care under the supervision of a registered nurse or licensed practical nurse. Discharges duties in a responsible professional manner consistent with all standards of nursing care, hospital policies and procedures, and the current regulations governing nursing practice in the state.

Requirements

  • High School Diploma or equivalent.
  • Obtain certification in Basic Life Support within 30 days of hire date.
  • Graduate of a nurse assistant training program OR Nursing Student.

Nice To Haves

  • Certified Nursing Assistant through the West Virginia DHHR.
  • One (1) year of clinical experience.

Responsibilities

  • Follows established plan of patient care.
  • Assists with the admission, transfer, discharge and transporting of all patients.
  • Implements patient safety measures including: Secures patients appropriately before transporting. Assists patient in moving, sitting and ambulating. Raises side rails as indicated. Implements restraint alternatives. Applies restraints under the direction of registered nurse. Implements fall protocol under direction of registered nurse. Ensuring call light is within easy reach. Applies monitor leads (Lead II) only.
  • Collects and records vital signs as instructed by the nurse.
  • Collects and prepares specimens according to policy and procedure including but not limited to: urine, stool, sputum, and/or glucometer and record results in patient’s chart (Not to be done on SCU) Administers care and treatments for patients as instructed by the nurse including: Patient weights Gives enema Bathing (am/pm care) Performs perineal care Performs catheter care Performs oral hygiene Applies hot or cold compresses Strains urine Obtains T.P.R./Blood pressures (No blood pressure on SCU) Obtains Intake and output and document results in patient’s chart Ambulates patients Turns and positions patients Provides Sitz bath Provides Pre‑operative care Provides Post‑operative care Performs Post‑mortum care Applies assistive devices, braces and equipment Traction set-up and application Performs 12-lead EKG (Not to be done on SCU) Performs oral suctioning with yankers catheter
  • Performs basic routine duties including: Distributes fresh water every shift and as needed Disposes of soiled linen every shift Maintains clean and orderly unit Maintains and stocks unit supplies Practices clean and sterile technique in the delivery of patient care while maintaining Body Substance Isolation. Aware of the location of equipment and supplies on the nursing unit.
  • Demonstrates knowledge and skill necessary to provide care appropriate to the age of the patients served. (Adolescent 13-17 years, Adult 18-79 years, Geriatric 80+ years) Understands the basic growth and development stages. Gathers information relative to patient's status to identify the age specific needs of each patient and reports this to the nurse. Maintains an understanding of treatment necessary for each patient and provides appropriate care based on their age specific needs.
  • Performs food and nutritional duties including: Distribution of snacks and supplements. Maintains fluid restrictions. Assists patients with meals and supplements as needed. Fills out caloric count. Marks meal percentage. Passing of trays. Collection of trays.
  • Performs 12-lead EKG within one hour of MD’s order: Maintains resident’s dignity/privacy during procedure and places electrodes correctly on chest wall.
  • Performs daily documentation.
  • Provides care for the respite and hospice patient.
  • Maintains two-way communication in a professional manner including the following: Keeps nurse informed about department activities or problems. Establishes and maintains good rapport with hospital personnel. Establishes and maintains good rapport with patients and families. Explains tasks to patients. Provides emotional support to patient and family.
  • In-services and other educational programs as required.
  • Attends and participates actively in a minimum of three staff meetings annually.
  • Demonstrates proficiency, competence and current knowledge in providing patient care.
  • Determines areas of self-improvement; writes goals on performance appraisal.
  • Responds to patient's signal light immediately; notifies nurse of patient problems.
  • Provides patient care in a manner that promotes maintenance or enhancement of the patient’s quality of life.
  • Reports observations, requests and unusual signs and symptoms of patient to charge nurse or team leader immediately.
  • Supports and adheres to hospital and nursing policies and procedures.
  • Institutes emergency measures calmly and effectively.
  • Reports any safety or risk management problems to charge nurse or appropriate hospital personnel.
  • Documents technical skills performed, including observations, in the medical record.
  • Provides the appropriate educational information to the patient and his/her significant other throughout the hospital stay.
  • Assists with preceptorship program when required.
