About The Position

This Float Licensed Practical Nurse (LPN) position is on a PRN (as needed) basis, requiring flexibility with scheduling and travel to support staffing needs across multiple communities. The role involves overseeing medication distribution according to physician orders and community procedures, creating personalized service plans based on resident assessments, fostering communication between physicians, healthcare providers, and families regarding resident well-being, contributing to quality assurance and performance improvement, and engaging in continuous learning and development.

Requirements

Current state licensure as Licensed Practical Nurse (LPN)
Must hold valid driver's license as travel may be required
Current CPR certification
Exceptional communication and interpersonal skills
Must demonstrate the ability to serve with love, compassion, and dignity

Responsibilities

Demonstrate flexibility with schedule and travel to support staffing needs across multiple Communities
Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety
Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity
Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well-being of our residents
Contributes toward quality assurance and performance improvement initiatives
Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experience
