About The Position

This position is for a Registered Nurse (RN) in Missouri, operating as a hybrid role that requires fieldwork to visit patients’ homes in Kansas City, MO, and surrounding areas, with the possibility of occasional commuting to the Kansas City, MO office. The Personal Care Services department provides caregiving arrangements where a patient has a principal caregiver offering daily support based on their care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in their home, offering a range of care options as their needs evolve. This service is designed to provide alternatives for long-term care to persons who meet nursing facility-level care needs, while preserving the dignity, self-respect, and privacy of the patient. In this role, as a licensed practical nurse, you will perform clinical evaluations in the home of Personal Care Services members to determine their qualification for the program and conduct monthly visits with members who are On Care with FreedomCare to ensure quality service and address any changes in their condition.

Requirements

  • Licensed Registered Nurse licensed by Missouri Board of Nursing.
  • Minimum of 2 years clinical experience
  • 1 year of experience with elders or adults with disabilities
  • Valid driver’s license with an insured vehicle for travel within the state
  • Understanding of regulatory and provider compliance requirements

  • Strong communication and interpersonal skills.
  • Problem-solving and decision-making abilities.
  • Organizational and multitasking skills.
  • Excellent communication, critical thinking, and organizational skills

Nice To Haves

  • Bilingual in Spanish is a plus
  • Experience with CRM (Salesforce is a plus)

Responsibilities

  • Provides support to the caregiver and member, and reviews services according to scope of the Personal Care Services program.
  • Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.
  • Follows the FreedomCare’s clinical policy and procedures.
  • Assesses and regularly reassess the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.
  • Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.
  • Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
  • Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.
  • Interprets the philosophy and objectives of the agency to clients, families, and others.
  • Performs other job-related duties as may be required or directed by the supervisor.
