About The Position

The Field Case Manager will meet with assigned clients in their home or community to support clients in gaining or maintaining access to permanent housing and support services to promote permanent housing stability, self-sustainment, assistance with medical, legal, psychiatric care, and employment.

Requirements

  • Bachelor’s Degree in related field or 2-3 years case management experience
  • Registered RADT or AOD Certification preferred.
  • SOAR trained (Supplemental Security Income/Social Security Disability Insurance Outreach, Access, and Recovery). Training can be completed upon hire.
  • Responsible for maintaining professional certification(s), if applicable
  • Basic understanding of the homeless veteran population.
  • Knowledge of HMIS databases a plus.
  • Ability to prioritize competing responsibilities and thrive in challenging situations.
  • Excellent time management skills
  • Ability to be reliable, an active listener, and a problem-solver
  • Must be able to fluently read, write and communicate in English.
  • Must comfortably be able, both mentally and physically, to perform all aspects of job description
  • Perform other duties as assigned by supervisor.
  • Ability to work in the community with clients.
  • Current CA driver’s license, in good standing
  • Ability to bend and lift light objects
  • Ability to sit for extended periods of time while viewing a computer monitor
  • Must be able to work in a noisy, busy environment.
  • Must be able to drive to destinations throughout San Diego County

Responsibilities

  • Meet with assigned clients at home or in the community regularly to monitor housing stability, assess progress, formulate treatment plans, and provide education, resources, and case management.
  • Coordinate/facilitate community support services as appropriate including but not limited to legal, food, financial, clothing, and employment, as well as medical, mental health and substance abuse services.
  • Support housing search and placement activities to facilitate transition to or sustainability of permanent housing and self-sufficiency.
  • Support clients with building resources in the community.
  • Review and manage assigned case files.
  • Documenting with Program, County and Grant guidelines
  • Maintain minimum caseload.
  • Provide outreach to clients that have been referred to program.
  • Develop resources and networking in the community
  • Attend all required meetings and complete all mandated company trainings on time as prescribed.
  • Perform crisis intervention.
  • Other duties as assigned by supervisor.
