Wellness Coordinators function in a non-clinical capacity responsible for providing care coordination or administrative support to members with behavioral health and substance abuse needs. Wellness Coordinators will be a primary care navigator by locating and engage members, screening for and identifying gaps in care, addressing SDOH needs and developing individual care plans to overcome barriers to access to care. Wellness Coordinators will also provide administrative support to the Behavioral Health Case Management department through assigning referrals, providing chart summaries and verification of member demographics. Working Schedule: Schedule is Monday through Friday from 8:00 am to 5:00pm Pacific Standard Time. This position is a field-based position with a home-based office. You will work from home when not in the field Location: West Seattle/Smokey Point-3955 156th St NE, Marysville, WA 98271. Local travel up to 25%25 and mileage is reimbursed at current government rate. If you reside within the state of Marysville, WA, you will enjoy the flexibility to work in both the field as well as remotely as you take on some tough challenges. Primary Responsibilities: Wellness Coordinators work in an inbound, but mostly outbound member queue Handle inbound calls from behavioral health providers in a dynamic call queue environment. Efficiently navigate and use multiple computer databases and screens during calls. Collaborate with clinical partners (Care Advocates) across various levels of care as required Maintain compliance with departmental metrics and quality standards. Ability to navigate through potentially sensitive and traumatic member history and handle crisis situations Documentation in multiple electronic documentation systems Coordinate information to clinical team partners Provide behavioral health benefit education Contact members to screen for and identity gaps in care Provide network referrals and appropriate community resources to help the member engage with mental health and substance use treatment Administrative support 25%25 Field work to nearby inpatient Behavioral health Facility Performs other duties as assigned What are the reasons to consider working for UnitedHealth Group?
