About The Position

POSITION SUMMARY: THE FELLOW IS RESPONSIBLE FOR CLINICAL DUTIES (4 SESSIONS PER WEEK) PLUS ACCOMPANYING PATIENT CARE ADMINISTRATIVE WORK. ESSENTIAL DUTIES AND RESPONSIBILITIES: Maintains working knowledge of and adhere to the program Manual, agency handbook and Policies and Procedures Commitment to providing cost conscious, fiscally responsible, timely, efficient, effective, safe, patient-centered, culturally competent, equitable, and participatory health care. Attends all required trainings as per Agency Policy, Program Manual and Funder Guidelines. Arrives on time for work, remains at work for whole shift, regular and predicable attendance. Maintains appropriate boundaries and ethical standards with staff, clients, visitors and outside providers as detailed in Agency Policy. Maintains positive and professional working relationship with co-workers, managers, clients, visitors and outside providers  Strong and clear written and verbal communication skills Excellent computer skills (included but not limited to Microsoft Word, Excel and Outlook) Embodies the following values as set forth in the strategic plan:  Compassion, Knowledge is power, equity and inclusion, excellence, courage and capacity to change. Any and all other duties as assigned, including academic expectations for the Fellowship.    PROGRAM SPECIFIC ESSENTIAL DUTIES AND RESPONSIBILITIES:  Collaborates with overall practice management, monitors special populations as part of the HIV team and acts as a consultant to other Medical Providers.  Participates in Quality Improvement (QI) projects in collaboration and coordination with the HIV team.    Assists in coordinating team-based QI projects.  Any and all other duties as assigned.

Requirements

  • Physician, Nurse Practitioner, or Physician Assistant as their primary job description.
  • Prior training in primary care (MD/DO residency, NP residency, or clinical experience).
  • Familiarity with clinical practice in Patient-Centered Medical Home model of care required.
