About The Position

Physical Therapist Fee for Service Now Hiring for FFS Physical Therapists in Brooklyn, Queens and Manhattan! Your Choice At Home is an affiliate of Cobble Hill Life Center and a Certified Home Health Agency (CHHA). We're culturally sensitive, and many of our staff come from within the community that they serve. Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services. As a Home Care Physical Therapist, you will be responsible for evaluating the client's level of physical functioning in the home and for working with the client, family, and/or others for the planning, provision, and coordination of physical therapy services as per MD prescription. You'll also work with other professional staff to establish rehabilitation and maintenance goals and activities.

Requirements

• License and current registration to practice as a Physical Therapist in New York state.
• Graduate from a physical therapy curriculum approved by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association.
• A minimum of 1-year experience in a hospital or long-term care facility as a Physical Therapist
• Experience working with a pediatric population is required.
• Demonstrated knowledge of Medicare/Medicaid, regulatory, and credentialing criteria and standards.
• Access to reliable transportation and able to make home visits in assigned service area(s)

Nice To Haves

• A combination of experience working with pediatric, adult, and geriatric populations is a plus.
• Prior experience as a home care Physical Therapist a plus

Responsibilities

• Developing a plan of treatment that encompasses treatment modalities, frequency of visits, expected outcomes and addresses short- and long-term goals.
• Implements the plan of treatment under physician direction and in collaboration with agency professional personnel.
• Observes records and reports to the physician and program personnel the patients' reaction and treatment and any changes.
