Requirements

  • Bachelor’s degree in social work or in a related human service field required.
  • Background check clearance, both criminal and CANTS required.
  • State licensing/certification as required by the State of Illinois Department of Children andFamily Services.
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
  • Valid driver’s license, in good standing for the state of residency required.
  • Access to reliable transportation.
  • Valid IL statutory minimum liability insurance coverage, bodily injury and property damagerequired.

Nice To Haves

  • Experience in social service work preferred.
  • Experience working with schools, healthcare facilities and government institutions preferred.

Responsibilities

  • Coordinate and collaborate with other service providers, schools and healthcare facilities topromote and ensure child well-being and safety during service delivery.
  • Assess family service needs and provide linkage to community resources to remediateproblems and prevent abuse or neglect and verify participation.
  • Provide strengths-based case management to facilitate the resolution of low to minorchallenges contributing to risk to children.
  • Advocacy with medical or governmental institutions.
  • Complete and submit all required reports, documents, and billing accurately and timely asrequired by the agency and funding sources.
  • Participate in supervisory process for guidance, direction support, and approval of decision-making,assessment, service planning and intervention.
  • Work in collaboration with the CCS Network Management Team and/or other intra agencyentities on programmatic, fiscal, community and organizational issues.
  • Consistently refine practice to achieve outcome measure established by the organizations Continuous Quality Improvement Process as well as contractual and program plan requirements.
  • Demonstrate sensitivity and responsiveness to cultural differences present within the agency and with clients.
  • Respond to all incoming correspondence and questions in a timely manner, including but not limited to inquires received via email, telephone, text message, and fax
  • Ensure reasonable measures are taken to protect confidential and/or sensitive information as well as ensuring the security of all LSSI systems is maintained.
  • Responsible for completing all required training assignments as well as complying with the related organizational, administrative, and programmatic policies.
  • Perform all other duties and tasks as assigned.
