About the Role As a Supportive Housing Case Manager, you will support families in achieving safe housing, economic security, and healthy family functioning. You’ll work closely with parents, children, and partner agencies to ensure stability, safety, and long-term success—always with the goal of family preservation and reunification. This role is ideal for someone who is organized, compassionate, and committed to empowering families navigating challenging circumstances.

Requirements

  • Strong case management skills with experience supporting families.
  • Ability to coordinate multiple services and work collaboratively with partners.
  • Strong organizational, communication, and advocacy skills.
  • Flexibility to work evenings or weekends as needed.
  • Reliable transportation (use of personal vehicle required).
  • Bachelor’s degree required.
  • Knowledge of child welfare, substance abuse, welfare, or supportive housing system.
  • Knowledge of culture of addiction.
  • Must be proficient in Microsoft 365.
  • Must be able to navigate through electronic health records system (EHR).
  • Must be able to obtain and maintain the certified domestic violence counselor certification per CGS 52-146k before providing direct service to clients, followed with six-hour recertification yearly
  • Must have and maintain a valid driver’s license.
  • Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.

Responsibilities

  • Conduct client intakes, assessments, and evaluate program eligibility
  • Develop individualized care plans and monitor client progress
  • Provide advocacy, crisis intervention, and referrals to supportive services
  • Coordinate care with DCF Social Workers, treatment providers, and community partners
  • Conduct home visits, including unannounced safety checks
  • Support clients in accessing services such as substance abuse recovery, parenting education, job training and financial management
  • Assist with lease-up processes for Family Unification Program vouchers
