The Support Worker will be a full and integral member of the team, providing formalised support and practical assistance to service users in helping them to achieve goals of their person centred care plan. To facilitate and support each service user in order to help him/her achieve personal goals identified on his/her individual programme in order to live independent lives. They will facilitate and support information sharing to promote choice, self-determination in line with the New Directions Personal Supports for people with Disabilities. As a core member of the team, the Support Worker will work alongside an agreed number of service users on a one to one and / or group basis. They will also co-work with their colleagues and will work under the supervision of line management. The Support Worker will take a lead role in empowering the service user(s) to live an independent life.
