As the Family Support Specialist you will be responsible for providing the supportive services which enable families to achieve self-sufficiency and reach their potential in Linn, Johnson, Washington, Benton and Iowa Counties. These activities include Family Support, skill development, life skills advocacy, and care management implementation. Specific responsibilities include:

Requirements

  • To qualify for this position you must meet one of the following criteria:
  • Possess a Bachelor’s Degree in a Social Services related field of study (or)
  • Possess a Bachelor’s or Associate’s Degree in a human services related field of study (or)
  • Possess a High School diploma/GED and the equivalent of 5 years of full time experience in social services or other human services delivery.
  • Must be at least 23 years of age
  • Strong interpersonal and communication skills
  • Effective problem solving and coordination abilities
  • Basic computer skills in a MS Office environment

Responsibilities

  • Complete home visits with families on a monthly basis
  • Work with the whole family with a supportive and non judgmental mindset
  • Involve the families in goal setting that is important to them, work through barriers to achieving goals
  • Complete required assessments child development, relationships, support systems, brain health
  • Identify community resources available and how to access
  • Mediate conflicts and problem-solves breaking of real or perceived barriers to the child/family’s involvement in the community.
  • Network with community service representatives to advocate for accessibility of non-traditional resources for families.
  • Document service delivery, including objectives of treatment and intervention strategies.
  • Facilitate community resource planning and case progress evaluation
  • Participate in discharge planning & documentation.
  • Identify community resources available to support families.
  • Work with community resources, vendors, & public systems to achieve self-reliance.
  • Establish relationships & counsels families to support independence.
  • Report on client activity and community contacts.
  • Teach basic living skills, including financial management & employment skills.
