About The Position

Position Highlights: Quality of Life: Where career opportunities and quality of life converge Advancement: Strong orientation program, generous tuition allowance and career development What you will do: The Family Support Specialist creates partnerships with parents and their families to promote child well-being and prevent the abuse and neglect of children through intensive home visiting. Staff receive intensive and ongoing training related to trauma informed practice. By strengthening the parent-child relationship by improving parenting skills, reducing family stress, and meeting the needs of parents.

Requirements

• HSD/GED Required.
• Must be Fluent in Spanish required.
• Relevant experience working with children and families
• A valid NYS driver's license required.
• valid automobile insurance is required.
• Can potentially travel up to 400 miles a month in both rural and urban communities and are reimbursed for mileage by the set federal reimbursement rate.
• Pay Range: $17.85 - $22.20 Pay is based on experience, skills, and education.

Responsibilities

• Establishes a trusting relationship with at-risk families by providing regularly scheduled home visits from intake through enrollment up to 5 years for each family as determined by HFA Accreditation Standards.
• Participates in weekly mandatory supervision, with a focus on administration, Clinical and reflective strategies.
• Ensure accurate and timely reporting and case documentation in written or electronic Healthy Families NY database and management information systems, within the required time frames.
• Maintain at least a 75% home visit rate as determined by HFA Best Practice Standards and maintain a case load of up to 25 families.
• Provide continuous outreach to families and community referral sources to maintain caseloads and program capacity as determined by Office of Children and Family Services NYS contract.
• NYS mandated reporters and must follow HFNY policies and procedures related to the NYS law for mandated reporters which includes mandatory annual training.
• Functions as a representation of HFRC/SPHP at community wide events and works collaboratively with community agencies.
