About The Position

Requirements

  • Education: High school diploma, Bachelor Degree preferred
  • Experience: Minimum of one (1) year in maternal and child health or related field, experience working with, or assisting, at-risk children and families in a community setting. Familiarity with the targeted population and geographic location of the assigned program. Knowledge and experience relating to the field of non-profit and education is strongly preferred
  • Language: must be bilingual in English and Spanish
  • Computer proficiency required with Microsoft programs. Strong interpersonal & oral/written communication skills required.
  • Valid Driver’s license with the ability to provide proof of ongoing automobile insurance coverage is required. This position will entail reimbursable local and statewide travel utilizing personal vehicle.

Responsibilities

  • Conduct initial and ongoing assessment of pregnant women and children to identify eligibility in home visitation supportive services.
  • Conduct weekly home visits with pregnant women and new mothers in the target area, to provide education, care management and supportive services.
  • Link and refer women of childbearing age, infants, and children to services as needed.
  • Perform home assessments to determine needs of families.
  • Reconnect women and children, who have been identified as “lost to care”, to necessary medical and social services.
  • Identify and address barriers to care.
  • Outreach to families and community agencies to provide linguistically appropriate health information, to heighten awareness of program services and community resources.
  • Perform other related duties as requested by the Program Supervisor.
