About The Position

A child welfare program providing intensive case management, vocational supports, and temporary housing subsidies to families that are homeless or at risk of homelessness in order to support the achievement of family reunification and/or prevent the removal of children from their families. POSITION: Family Support Specialist 1 Are you interested in playing a pivotal role in the lives of families and children? If you are, come join the exciting Supportive Housing for Families team as a Family Support Specialist! In this role, you will support clients in achieving their individual goals by providing intensive in-home case management services. You will help families address the problems of homelessness, unemployment, mental illness, and substance use through collaboration with treatment providers, natural supports, and community resources. Requirements

  • Motivation, passion, and commitment to the Mission of The Connection
  • Exceptional interpersonal and communication skills with ability to foster effective working relationships
  • Strong case management experience and skills
  • Bachelor’s Degree and 2 years of relevant experience (minimum)
  • Valid Driver’s License

Nice To Haves

  • Previous experience with Motivational Interviewing (preferred)

Responsibilities

  • Weekly in-home visits targeted to support clients in developing and achieving goals to find housing, employment, and treatment options.
  • Identify resources and facilitate referrals, as needed, for housing, eligible benefits, substance use, mental health, counseling, child specific services, life skills including budgeting, vocational and educational opportunities.
  • Regular and proactive collaboration with stakeholders such as DCF, treatment providers, landlords, internal staff, and departments.
