Long-Term Services and Supports (LTSS, sometimes known as Long-Term Care) is a broad term for many health care programs available to help people who have disabilities or older adults with their daily activities. LTSS includes Home and Community Based Services (HCBS), Community First Choice (CFC) services, and State General Fund programs which have different requirements to qualify. A Case Management Agency (CMA) assists individuals and families with accessing necessary HCBS Waivers, CFC services, and other supports to meet their needs. The CMA recently underwent an internal redesign. Part of the intake case management team merged with the Family Support Services Program (FSSP) to create a hybrid role called Intake Navigator. This team coordinates case management, care planning, and referrals for individuals eligible for Colorado’s Medicaid Waivers and FSSP, in alignment with federal requirements. This role is responsible for member-facing case management, including receiving and processing HCBS referrals for IDD, Non-IDD, and FSSP Waivers and programs, conducting eligibility screen tools and assessments, determining developmental delays or disabilities, working with our LTC Financial Program Partners to establish and confirm financial eligibility, connecting members to community resources, developing FSSP Service Plans, and monitoring FSSP Funding needs. The position requires strong knowledge of community resources, policies, and regulations, as well as the ability to advocate effectively for members. Experience in a high-volume, customer-focused environment is essential, with the ability to manage competing priorities, respond to inquiries, and handle complex situations with professionalism and empathy.
