About The Position

Long-Term Services and Supports (LTSS, sometimes known as Long-Term Care) is a broad term for many health care programs available to help people who have disabilities or older adults with their daily activities. LTSS includes Home and Community Based Services (HCBS), Community First Choice (CFC) services, and State General Fund programs which have different requirements to qualify. A Case Management Agency (CMA) assists individuals and families with accessing necessary HCBS Waivers, CFC services, and other supports to meet their needs. The CMA recently underwent an internal redesign. Part of the intake case management team merged with the Family Support Services Program (FSSP) to create a hybrid role called Intake Navigator. This team coordinates case management, care planning, and referrals for individuals eligible for Colorado’s Medicaid Waivers and FSSP, in alignment with federal requirements. This role is responsible for member-facing case management, including receiving and processing HCBS referrals for IDD, Non-IDD, and FSSP Waivers and programs, conducting eligibility screen tools and assessments, determining developmental delays or disabilities, working with our LTC Financial Program Partners to establish and confirm financial eligibility, connecting members to community resources, developing FSSP Service Plans, and monitoring FSSP Funding needs. The position requires strong knowledge of community resources, policies, and regulations, as well as the ability to advocate effectively for members. Experience in a high-volume, customer-focused environment is essential, with the ability to manage competing priorities, respond to inquiries, and handle complex situations with professionalism and empathy. Requirements

  • Experience: Three (3) years of work-related experience
  • Education: Bachelor’s Degree in a related field
  • Note: An equivalent combination of education and experience is acceptable.

Responsibilities

  • Receives and screens all incoming referrals, including FSSP referrals.
  • Provides information about programs and services, eligibility criteria, and steps for accessing services.
  • Sends appropriate application forms upon request and provides assistance in completing forms and collecting required documentation, including referrals to evaluation specialists.
  • Determination of developmental delay or developmental disability is made within required timelines.
  • Ensures consumers or guardians receive written notification of results within established timeframes.
  • Tracks all applications in designated data systems according to established timelines and ensures required information is entered in the State data system for reimbursement.
  • Maintains intake and master records in compliance with regulatory requirements.
  • Enrolls eligible individuals and families in the FSSP.
  • Scores the FSSP Most-In-Need Assessment, identifies priority needs and concerns, and develops service plans authorizing services and funding within program parameters.
  • Maintains member care plans by reassessing as necessary and provides ongoing case management services.
  • Screens individuals for potential eligibility for Medicaid Waivers and, when appropriate, initiates the waiver enrollment process, including explaining application steps, required documentation, and coordinating referrals to Waiver Enrollment Case Managers.
  • Works with various Medicaid programs, including Home and Community Based Services (HCBS), Elderly, Blind and Disabled, Community Mental Health Services, Brain Injury, Children’s HCBS, Program of All-Inclusive Care of the Elderly (PACE), Home Care Allowance, and Nursing Home programs.
  • Communicates and collaborates with medical providers, state agents, and community partners to promote coordinated care.
  • Ensures members and families are fully informed of available options and makes referrals to appropriate programs and services, providing follow-up as needed.
  • Coordinates FSSP transfers when individuals move within Colorado, ensures all due process protections are met, and completes necessary documentation.
  • Prepares legal documents for appeal hearings and attends court hearings to defend denied applications.
  • Understands characteristics of family risk and supports development of plans that build caregiver capacity.
  • Establishes trusting and respectful relationships with families through culturally responsive and inclusive practices.
  • Performs other duties and responsibilities as assigned.
