About The Position

The Center for Human Development, (CHD) is seeking a Caseworker to join our Diversion, Shelter and Housing (DSH) program working at our Shelter in Holyoke, MA. This case worker will be working with people who are experiencing homelessness/living outside to connect them with resources and find shelter and/or housing for them. The Caseworker will provide triage case work and outreach. Your role as a Caseworker: The Caseworker is a paraprofessional who actively supports the philosophy of the agency and maintains a high level of commitment to participants served. The Caseworker provides a supportive relationship with clients in need of services that address needs regarding mental health, substance use, and housing and facilitates their living independently in the community.

Requirements

  • Experience working with psychiatric disorders or co-occurring population preferred
  • Bachelor’s degree from an accredited educational institution preferred
  • Substance abuse and mental health experience preferred
  • Multiple years of experience in case management and social services preferred
  • High school diploma or GED required
  • Required valid Massachusetts driver’s license with ability to pass background driving record check
  • Required vehicle readily available on site and throughout all work hours
  • Approved criminal background check (CORI)
  • The Individual Caseworker should actively support the philosophy of the agency and maintain a high level of commitment to program participants and staff
