About The Position

The Family Strengthening Facilitator will provide emotionally and developmentally based assessment, support, case management and activities to the children and families that qualify for Broward Children’s Service Council (CSC) Family Strengthening Program of Broward Children’s Center. The goal of this role is to promote effective coping skills along with cognitive and psychosocial growth in the families served.

Requirements

  • 4 Year Degree
  • Knowledge of and an ability to use various computer programs such as word processing, spreadsheet, calendar and email.
  • Effective verbal and written communication skills.
  • Be able to complete thorough psychosocial assessments and other evaluations.
  • Be able to relate to children and families in supportive and therapeutic ways.
  • Be able to effectively assess and create effective treatment plans.
  • Demonstrate knowledge and skills necessary to provide care based on physical, psychological, developmental, social, educational, safety and related criteria, appropriate to the age of the clients served.
  • Valid Florida Driver's License.
  • HIV and HIPAA Course.
  • CPR and First Aid.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Responsibilities

  • Complete all required documentation appropriately and in a timely manner.
  • Complete chart abstractions and internal quality assurance of charts to meet yearly monitoring requirements.
  • Assure that the program is in compliance with contractual requirements, utilization of program units, funding, Company policies and procedures and all applicable state and federal laws.
  • Review referrals and initiate communication with families in a timely manner to develop a strong rapport with families.
  • Effectively lead family and group discussions providing support and counseling techniques to accomplish Nurturing Program goals and objectives.
  • Maintain records, including attendance, evaluations and assessments as well as additional activities conducted, program enhancements, and tips to be used to enriched the program in the future.
  • Obtain necessary consents as needed.
  • Institute individualized service plans of activities and/or behavioral strategies as needed for families and children with the support of the interdisciplinary team.
  • Provide interventions, strategies and techniques to meet the needs of the children and to foster positive growth and development within the family.
  • Increase the awareness of services throughout the facility and community by providing information and education about the services.
  • Assist clients to access appropriate benefits and basic needs; appropriate medical, mental health and substance use treatment providers.
  • Provide crisis intervention for immediate housing needs, medical, mental health and substance use issues.
  • Serve as resource and educator to interdisciplinary team and families.
  • Be flexible with working hours to meet the child and family’s needs, including evening (2-3 per week) and weekend hours as needed.
