About The Position

The Resource Case Coordinator is responsible for supporting individuals experiencing homelessness as they move through shelter, transitional housing, and ultimately into permanent housing. This position provides client orientation, ongoing case management, goal planning, referrals, documentation, and follow-up support. The coordinator works closely with clients to remove barriers, build life-skills, and connect them with community resources needed to achieve long-term stability.

Requirements

  • High School diploma or GED required.
  • Ability to pass a criminal background check, drug screening, and FBI fingerprinting.
  • Fingerprinting fee is the responsibility of the candidate.
  • Strong written and verbal communication skills using clear, understandable language.
  • Ability to work well under pressure, handle sensitive situations, and maintain professionalism.
  • Basic computer proficiency (email, case management systems, Microsoft Office).
  • Work is onsite in a shelter and residential setting.
  • Must be able to walk up and down stairs several times a day.
  • Ability to sit, stand, walk, and engage with residents for extended periods.
  • Ability to lift to 25 pounds occasionally.
  • May involve exposure to individuals in crisis, stressful situations, or behavioral challenges.
  • Must be able to maintain composure and utilize de-escalation techniques.
  • Willingness to consent to and the ability to pass a criminal background check, and drug screening.

Nice To Haves

  • Bachelor’s degree in human services, Social Work, Psychology, or related field preferred; equivalent work experience considered.
  • One (1) year of experience in case management, social services, or direct support preferred.
  • Knowledge of local community resources or willingness to develop community partnerships.

Responsibilities

  • Resident Support & Orientation
  • Welcome and orient new residents to shelter expectations, program rules, safety guidelines, and available services.
  • Explain program processes in clear, simple language.
  • Ensure residents understand confidentiality, conduct expectations, and available grievance procedures.
  • Case Management & Service Planning
  • Conduct intake assessments, identify strengths, barriers, and immediate needs.
  • Develop individualized case plans with measurable goals related to housing, employment, training, health, and life skills.
  • Meet regularly with clients (weekly or as required) throughout the program.
  • Provide continued case follow-up after transition to transitional or permanent housing for an extended period based on program requirements.
  • Make referrals to appropriate support agencies, including mental health services, employment programs, training, medical care, and benefits assistance.
  • Documentation & Recordkeeping
  • Maintain accurate case notes, progress updates, housing plans, and contact logs.
  • Document all behaviors, incidents, safety concerns, and client interactions in a timely manner according to agency standards.
  • Ensure compliance with confidentiality laws (HIPAA where applicable) and agency privacy policies.
  • Program Support & Collaboration
  • Participate in shelter meetings, case reviews, and multidisciplinary service planning.
  • Build and maintain relationships with community partners, resource providers, and housing agencies.
  • Support clients during housing applications, inspections, move-in coordination, and stabilization.
  • Professional Responsibilities
  • Manage deadlines and prioritize tasks in a fast-paced, high-pressure environment.
  • Maintain a trauma-informed, culturally sensitive, and client-centered approach at all times.
  • Participate in required training sessions including safety, crisis intervention, and program standards.
