The Family Health Center of Worcester (FHCW) School-Based Health Centers (SBHCs) are seeking a Family or Pediatric Nurse Practitioner to join our team. The Nurse Practitioner provides comprehensive, cost-effective, and high-quality outpatient health care to children, adolescents, and their families in a school-based setting. This role includes delivering diagnostic, treatment, and preventive services; managing chronic illnesses; and promoting health education for students and their families. Care is provided in alignment with the standards of advanced nursing practice and the mission of FHCW to improve access to equitable health care for all. The Family or Pediatric Nurse Practitioner works as part of a multidisciplinary care team that includes a Community Health Worker and Behavioral Health Clinician, collaborating closely with school staff to support the overall health and well-being of students. The Family or Pediatric Nurse Practitioner also serves as a vital liaison between the health center and the broader school community. Work Location and Schedule: This position is based at Claremont/Woodland Academy, within the Worcester Public Schools. Regular hours are Monday through Friday, 7:00 a.m. to 3:30 p.m., with occasional coverage at the Family Health Center of Worcester’s main site at 26 Queen Street. School-Based Health Center (SBHC) assignments may vary based on program and patient care needs. Any changes in site placement will be communicated with reasonable notice by the SBHC Director.
